Copy KTP/Kartu Pelajar, 1 sheet
Copy Ijazah, 1 sheet
Copy “Kartu Keluarga”, 1 sheet
Colored full length photo, 3R size with white background, smiling face, 1 sheet
Colored face photo, 4 x 6 size with white background, smiling face, 2 sheet
Materai 6.000, 4 (four) sheets
Scanned registration fee payment receipt Rp 200.000, –
Registration fee can be paid at Numyseed Cashier or transfer to:
